Step 1: Take Out the Claim Form

Make sure that you and the other party complete the front of the form in full. The form has carbon-copy sheets so that both parties can take a copy home. Fill in the back at home at your convenience.

Step 2: Fill Out the Front of the Claim Form Completely

The form contains 15 important sections you need to fill in:

Date and Time of the Collision

Record the exact date and time of the collision. Weather conditions can affect the accident and the claims process.

Location of the Collision

Enter the street and town/city where the collision took place. For motorways, include the number and the hectometer marker.

Injuries Sustained

Insurance Company Details

Enter the name, policy number, Green Card number, and validity period of the insurance.

Your Details

Enter your own details as the driver. Cyclists do not need to enter driver’s license details.

First Point of Contact

Indicate with an arrow where the vehicle was first struck.

Visible Damage to Your Vehicle

Describe all visible damage, such as dents, broken parts, or scratches.

Circumstances

Check the relevant situations and state the number of boxes checked.

Accident Sketch

Together with the other party, make a sketch of the accident. Draw the roads and vehicles, and indicate the direction and the point of impact.

Personal Explanation of the Accident Sketch

Briefly describe how, in your view, the collision occurred.

Signature

Read the form carefully and sign only if you agree with the contents. Do not let yourself be pressured into signing if you do not agree.

Step 3: Split the Form After Signing

Split the claim form in two so that both parties take a copy home, even if you have not signed the form.

Step 4: Fill in the Back of the Form Completely

Fill in the back of the claim form at home as soon as possible and include your IBAN number and injury details.

Step 5: Send a Copy to Your Insurance Company

Make copies of both the front and back of the form and send them to your insurance company. Keep the originals.
