Claim Form Completed Step by Step
Step 1: Get the Claim Form
Make sure that you and the other party fully complete the front of the form. The form has carbon copies so that both parties can take a copy home. Fill in the back at home at your convenience.
Step 2: Fill Out the Front of the Claim Form Completely
The form contains 15 important sections that you must complete:
Date and Time of the Collision
Record the exact date and time of the collision. Weather conditions can influence the accident and the claims process.
Location of the Collision
Enter the street and city/town where the collision occurred. For highways, include the number and the hectometer marker.
Injuries Sustained
Check yes if you sustained an injury, including mild initial symptoms. Record your complaints and seek medical advice when needed. Legal costs depend on liability and the agreed terms.
Property Damage
Select yes or no. If yes, provide the details of other damaged vehicles or objects.
Witness Details
Note the names, addresses, and phone numbers of witnesses.
Policyholder Details
Enter the details of the person who took out the insurance, for example the owner of the vehicle.
Vehicle Details
Enter make, model, license plate, and country of registration. If there is a trailer, also provide its details.
Insurance Company Details
Enter the name, policy number, Green Card number, and validity period of the insurance.
Your Details
Enter your own details as the driver. Cyclists do not need to provide driver’s license details.
First Point of Impact
Indicate with an arrow where the vehicle was first hit.
Visible Damage to Your Vehicle
Describe all visible damage, such as dents, broken parts, or scratches.
Circumstances
Check the relevant situations and indicate the number of boxes checked.
Situation Sketch
Together with the other party, make a situation sketch of the accident. Draw the roads, vehicles, and indicate the direction and the point of impact.
Personal Explanation of the Situation Sketch
Briefly describe how, in your view, the collision occurred.
Signature
Read the form carefully and sign only if you agree with the contents. Do not let yourself be pressured into signing if you do not agree.
Step 3: Split the Form After Signing
Split the accident report form in two so that both parties take a copy home, even if you have not signed the form.
Step 4: Complete the Back of the Form in Full
Fill in the back of the accident report form at home as soon as possible and include your IBAN number and details of any injuries.
Step 5: Send a Copy to Your Insurance Company
Make copies of both the front and back of the form and send them to your insurance company. Keep the originals.
Filling Out the Accident Report Form Without the Other Party
